History Of Enema
Etymology
Enema came into English about 1675 from Latin, where it was first used in the 15th century to refer to a rectal injection, from Greek (énema), “injection,” itself from (enienai), “to send in, inject,” from (en), “in” + (hienai), “to send, throw.”
Clyster came into English in the late 14th century from Old French or Latin, and is derived from the Greek (klyster), “syringe,” which is derived from (klyzein), “to wash out.” It is a historically obsolete term for enema, more specifically for enemas given using a clyster syringe. It was also pronounced glister in the 17th century, though it is rarely cloiste” or clister.
Medieval and ancient history
Africa
The Ebers Papyrus (circa 1550 BCE) includes the first mention of the enema in medical literature. An Iri, the Shepherd of the Anus, was one of the many forms of medical specialists. Enemas were used to prescribe a variety of drugs. A Keeper of the Royal Rectum may have primarily served as the pharaoh’s enema creator. According to Egyptian mythology, the enema was created by the god Thoth.
The calabash gourd is historically used to administer enemas in parts of Africa. On the Ivory Coast, the narrow neck of a gourd filled with water is inserted into the patient’s rectum, and the contents are then injected by forcible oral inflation by an attendant, or a patient may self-administer the enema by using suction to create a negative pressure in the gourd, placing a finger at the opening, and then removing the finger to allow at least one anal insertion. Along the upper Congo River, an enema apparatus is created by drilling a hole in one end of a gourd and attaching a hollow cane to the gourd’s neck with resin. The cane is placed into the patient’s anus, which is in a position that enables the fluid to be infused by gravity.
The Americas
The Olmec used trance-inducing substances ceremonially from the middle preclassic period (10th to 7th centuries BCE) until the Spanish Conquest, and these were ingested through, among other things, enemas administered in jars.
Mayan sculpture and ceramics from that time depict scenes in which ritual hallucinogenic enemas were injected using syringes made of gourd and clay. The Maya demonstrated the use of a distinctive enema bulb syringe by female attendants administering clysters ritually in the Xibalban court of the God D, whose worship included ceremonial cult paraphernal.
Enemas were also used by the Mayans to treat sickness and digestive pain, as recorded in the Florentine Codex during the colonial era.
To induce respiration, indigenous peoples in North America used cigarette smoke enemas, which were administered through a rectal tube.
The indigenous peoples of South America used a rubber bag attached to a conical nozzle as an enema syringe at one time, and the rubber enema bag with a connecting tube and ivory tip remained in use by them while a syringe was still the standard means of performing an enema in Europe.
Asia
Enemas were in use in Babylonia by 600 BCE, though it appears that they were first used because of a belief that an enema would drive the demon of illness out of the body. Enemas are mentioned in cuneiform inscriptions on Babylonian and Assyrian tablets dating from around 600 BCE.
Zhang Zhongjing was the first to use enemas in China, about 200 CE. “Collect a big pig’s bile and combine it with a small amount of vinegar.” According to Wu Lien-teh, his instructions are to “insert a bamboo tube three or four inches long into the rectum and inject the mixture.”
The enema syringe is one of 121 surgical instruments mentioned by Sushruta in India in the fifth century CE. The enema apparatus used by early Indian physicians was a bamboo, ivory, or horn tube attached to the scrotum of a deer, horse, or ox.
The “clyster-purse,” or collapsible part of an enema outfit made from ox skin or silk cloth and emptied by squeezing with the hands, is attributed to Avicenna (980-1037 A. D.) in Persia.
Europe
“If the previous food which the patient has recently eaten should not have gone down, offer an enema if the patient be healthy and in the prime of life, but if he be frail, a suppository should be administered, should the bowels be not well moved on their own accord,” Hippocrates (460-370 BCE) says.
“Treatment consists merely of three elements: drink, food, and the enema,” wrote Greek physician Asclepiades of Bithynia in the first century BCE. He also said that indigestion is caused by large food particles, and that the best cure was to consume the right amount of food and drink the right amount of wine, followed by an enema to get rid of the bad food.
The Greek physician Soranus suggested enemas as a healthy abortion procedure in the second century CE, and the Greek philosopher Celsus recommended a vitamin enema of pearl barley in milk or rose oil with butter for those suffering from dysentery and unable to feed, and Galen mentions enemas in many ways.
The earliest illustrations of enema devices in the Western world date from the Middle Ages, and they show a clyster syringe with a tube attached to a pump action bulb made of a pig bladder.
From the 15th to the 19th centuries, a simple piston syringe clyster was in use. The rectal nozzle of this system was attached to a syringe with a plunger rather than a bulb.
Western world in the modern era
Enema apparatus was mostly developed for self-administration at home beginning in the 17th century, and many were French since enemas were widely used in France.
Both midwives and man-midwives usually administered clysters to laboring mothers just prior to their birth, according to François Mauriceau’s early-modern treatise The Diseases of Women with Child, published in 1694.
When clyster syringes were used in Europe, the patient was put in the proper position (kneeling, buttocks raised, or lying on the side); a servant or apothecary would then insert the nozzle into the anus and press the plunger, resulting in the liquid remedy (generally, water, but also other preparations) being inserted into the colon.
Since a woman may be ashamed to expose her buttocks (and probably her genitals, depending on the position) to a male apothecary, some contraptions were invented that blocked all but the anal region from the apothecary’s view. Syringes with a special bent nozzle, which allowed self-administration and thus removed the embarrassment, were another innovation.
Clysters were used to treat constipation, as well as stomach aches and other ailments, with varying degrees of success.
Physicians became a popular target for satirists in the 17th century, resembling Molière’s caricature, whose prescription for everything was “clyster, bleed, purge,” or “purge, bleed, clyster.”
Clysters were a popular motif in late-eighteenth-century burlesque comedies.
Johann Jacob Woyts described an enema bag made from a pig’s or beef’s bladder attached to a tube as an alternative to a syringe in 1753.
Europeans started to emulate the indigenous peoples of North America’s use of cigarette smoke enemas to resuscitate submerged people in the 18th century. The Royal Humane Society of London issued tobacco resuscitation packages, which included a pair of bellows and a hose, which were located at different locations along the Thames. Furthermore, headaches, respiratory failure, colds, hernias, stomach cramps, typhoid fever, and cholera outbreaks were treated with enemas.
Up until the 19th century, clysters were a common medical treatment among the bourgeoisie and aristocracy of the Western world. Purgative clysters were used for a number of illnesses, the most common of which were stomach aches and constipation, due to the lack of medical expertise at the time.
Clysters were so common at King Louis XIV of France’s court, according to Claude de Rouvroy, duc de Saint-Simon, that the duchess of Burgundy had her servant give her one in front of the King (her modesty being preserved by an adequate posture) before heading to the comedy. He does, however, note the King’s and Mme de Maintenon’s surprise that she should take it before them.
The bulb enema was one of many new forms of enema administration equipment invented in the nineteenth century. Devices that enabled the solution to be infused by gravity, such as those used by South American indigenous peoples and the enema bag described by Johann Jacob Woyts, became common. These consist of a nozzle at the end of a hose that connects to a liquid-filled tank, such as a bucket or a rubber bag, that is placed or hanging above the receiver.
